Response Form
The Response Form provides you and your service with a clear and consistent framework that allow you to answer to any referral you receive from other practitioners (both accepted and rejected).
When to use
Use the Response Form to give clear feedback and guidance to practitioners after screening their referrals and to direct them to more appropriate support when necessary.

Streamline and speed up the process of activating support to citizens by equipping the referrer with clear information about what to do next and who to contact (when necessary)

Reduce inappropriate referrals by establishing a transparent decision making process that trigger a learning process for the referrer

How to use

With your team, use the Response Form every time you are responding to a referral.
When filling the form:

Provide comprehensive and clear information about the reason of your decision (positive or negative)

Clearly explain next steps and suggested actions to be taken by the referrer

If needed, indicate any appropriate service(s) and/or contact that might be helpful to the referrer and explain why
Embed the Response Form within your existing channels (e.g. email, messages, specific softwares).

Test the cards with some of the stakeholders before implementing the tool. Feedback will help you to refine contents.
