Referral Redirection
The Referral Redirection redirects the referrer to another more appropriate service and acts like a soft touch referral system between services by strengthening existing informal signposting practices.
When to use
Use the Referral Redirection Form to refer the case to another service if it is more appropriate for the presenting need.

How to use

Use the Referral Redirection Form every time a service rejects a referral because it should be taken up by another more appropriate service.

When filling the form, write down to whom the referral was originally made and to whom it should be redirected to.
​
Provide reasons to motivate the redirection.

Send the Referral Redirection Form to the other service along with the referral if the same referral could be handed over. If not, send the Referral Redirection Form back to the referrer and ask them to make another referral to the service redirected to, attaching the Referral Redirection Form.

Test it with some of the stakeholders/services before implementing the tool in your practice. Feedback will help you refine the way it works.
